An alleged lunatic was received into custody upon a medical certificate purporting to be in the form in Schedule 12 as prescribed by sec. 13 of the Lunacy Act 1890. That schedule requires, inter alia, that the persons "from whom" certain information with regard to insanity has been communicated must be stated.
